Health Rebates How to redeem a rebate from your Private Health Insurance

Health Insurance Fund of Australia (HIF)

Eligible under the ‘Healthy Lifestyle Benefits’ category.

When making a claim:
Practitioners need to provide an invoice stating their name, the patient's name and "Shake It Practitioner Weight Management consultation".

BUPA (NRMA, SGIO, SGIC, MBF, HBA)

Shake It is a BUPA approved weight management program under the banner of "Health Management" extras service. The benefit payable for BUPA is for the program as a whole.

Rebates are for face to face consultations only and the amount payable depends on your BUPA member policy.

When making a claim:
The Practitioner must include the words "Shake It Weight Management Program" on the receipt for the consultation. The Patient collectively sends the receipts to BUPA Claims at the end of their program and then receives their rebate in the normal manner. "Shake It Weight Management Program" will need to appear on the receipt.

St Luke’s Health

Weight management programs are paid under St Luke's ‘Health Management Program’ which are available to their members holding combined hospital and extras policies.

The Practitioner must quote the provider number allocated; A4000847, on any receipt or invoice.

HBF

Shake It is an approved ‘Healthy Living Program’.

When making a claim:
Practitioners need to include "Shake It Practitioner Weight Management Program” on the invoice for payment.

Health Care Insurance

Commonwealth Bank (CBHS) Health Fund

Practitioner must register with HCI before their client claims. Click here to download Weight Loss Provider Registration Application. Send the completed form to: Fax: 03 6432 1160 or email: enquiries@hciltd.com.au

The member can either send the claims in all at once or one at a time. Wording on the receipts need to contain the words "Consultation" and "Shake It".

The Practitioner must quote the provider number allocated; CBHS04725, on any receipt or invoice.

Note: refunds are subject to change, contact your health fund for further eligibility to claim.
